Please remember to bring your insurance identification card with you to your first appointment. Our team is happy to assist you with understanding your dental benefits and answering any questions you may have.

Our practice is not under contract with any dental insurance plans. We believe this allows us to focus on providing the highest quality, personalized dental care without limitations dictated by insurance companies. If your plan allows you to choose an out-of-network dentist, we will gladly submit your insurance claim on your behalf.

What Does “Out-of-Network” Mean?

Our office is not in-network with any dental insurance companies, which means we do not have contracts that limit how care is provided or priced by insurance carriers.

What this means for you as a patient:

    Most insurance plans allow you to visit an out-of-network dentist and still receive reimbursement for covered services.

  • We submit insurance claims on your behalf.
    As a courtesy, our team will file your dental insurance claims for you so you don’t have to handle the paperwork.

  • Insurance pays you directly.
    Your insurance company will send reimbursement directly to you based on your plan’s out-of-network benefits.

  • You receive personalized, high-quality care.
    Being out-of-network allows us to focus on what’s best for your oral health—not what an insurance company dictates.

  • No restricted treatment options.
    We are free to recommend the most appropriate materials, procedures, and treatment plans for your long-term dental health.

You can get a head start by filling out your New Patient Form prior to your appointment. Simply click the link below to download, complete, and print the forms. If your browser does not support PDF form filling, the forms may also be printed and completed by hand.

Important Insurance Notice

Please be aware that dental insurance is a contract between you and your insurance company. While insurance can help reduce out-of-pocket costs, all plans include limitations, exclusions, and maximums. No dental insurance plan covers 100% of recommended treatment.

We encourage patients to familiarize themselves with their individual policy provisions. Our team is always available to help explain how your benefits may apply to your care.
